Your Appointment

THINGS YOU SHOULD BRING TO YOUR APPOINTMENT

When you come for your appointment, please allow 2 hours with no interruptions to our meeting, and please bring the following things with you, if you have them:

• Motor Vehicle Collision Report (Police Report)

• Anything else from the police

• Your Driver's License

• Your Auto Insurance Card (in effect on the date of your MVC)

• Your Auto Insurance Declarations Page (if possiple)

• I will be checking for:

  • MedPay

  • Underinsured Motorist Coverage

• Any and all Health Care Insurance Cards including:

  • Medicare card

  • Medicare Secondary or Supplement or Replacement

  • Medicaid (MO Health Net or KanCare)

• All correspondence from your Insurance Company or the Tort Feasor's Insurance (the guy who hit you)

• Pictures of:

  • the damage to your car

  • the damage to the other car

  • the scene of the crash

  • any injuries to your own body (bruises, stitches, cast, etc.)

• Names of the passengers in your car and contact info

• Names of witnesses and contact info

• Names of all Heathcare Providers and contact info

  • Ambulance

  • ER Hospital

  • Follow-up Doctor/PCP

  • Doctor of Chiropractic

  • Physical Therapist, etc

• Any Medical Records or Bills that you have, including a list of everything you've paid out of pocket, including over-the-counter remedies

• Receipts of the above

• Diary or Journal you have written about how your healing has progressed

• Anything else you can think of that might be helpful​
